We are pleased to announce an exciting new partnership with Vantage Healthcare to provide onsite primary care services for residents of Ernest P. Barka Assisted Living Community.
Through this partnership, residents will have the opportunity to receive regular visits from a dedicated care team, including a physician and a nurse care coordinator—all from the comfort of home. In addition to primary care, the Vantage team will collaborate closely with our wellness staff, specialists, hospitals, and other healthcare providers to ensure quality care.
While participation in this service is completely optional, we encourage residents and families to explore the many benefits it offers. Having access to onsite primary care services means residents can receive consistent provider attention without the stress of arranging transportation and traveling to outside appointments. With regular weekly visits, the Vantage care team will become familiar with each resident’s history and preferences, resulting in more proactive preventative care. Additionally, coordination between Vantage and our existing staff will help reduce the risk of gaps in care, which can lead to improved health outcomes for residents.
